The Medication Management Services provided by The MPH Life ensure that older adults, families, and physicians can trust that their prescribed medications are properly managed and taken.
Our licensed Nurse Practitioner will arrange the medications as prescribed by the client's physician and work towards the goal of achieving safe self-administration. We work closely with the client's physician and pharmacist to maximize the effectiveness of the treatment plan and achieve positive outcomes.
The Moving Care Management service offered by The MPH Life aims to address the complex challenges faced by older individuals and their families who are contemplating a move or relocation. Our expert geriatric care managers evaluate the options available to determine the best course of action for the client.
After conducting a thorough assessment, the Care Manager will provide recommendations on whether a move is necessary and to what level of care. If relocation is deemed necessary, the Care Manager will make appropriate referrals based on the client's needs and preferences.
We can also arrange for moving specialists to handle the logistics of the relocation and post-move, the Care Manager will visit the older client to assist with the transition to their new environment.

Based on this assessment, the Care Manager recommends activities that can increase the client's sense of connection, joy, and fulfillment. This may include spending more time with family, re-engaging with a former spiritual connection, joining activities like cooking or traveling, renewing a creative outlet like art, connecting with nature, or getting a pet.
After the Quality-of-Life assessment, the Care Manager devises necessary solutions and implements monitoring of the solutions, if requested under this service. Our Personal Care Plus services are designed for independent seniors who require non-medical assistance with daily routine tasks such as hygiene, meal preparation, light housekeeping, errands, and shopping. In addition to providing a personal assistant, the "Plus" in Personal Care Plus denotes the involvement of a care manager who will oversee the care staff members employed by the private duty home care agency selected by the client.
The care manager will interview potential care staff members, and co-supervise their work to ensure high-quality care is provided. Monthly or as-needed home visits will be conducted by the care manager to assess and supervise the care provided by the staff members. Additionally, the care manager will write a monthly report and maintain open communication with the client as necessary.
